**Registered Nurse RN Clinical Supervisor for Home Care**

Job Description Summary

The Clinical Supervisor- Clinical Manager is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Supervisor- Manager is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel. The Clinical Supervisor- Manager oversees patient care services and personnel and makes patient and personnel assignments, coordinates care and referrals. There shall be one qualified full-time registered nurse supervisor of clinical services for each ten or fewer, full-time or full-time equivalent professional direct service staff assigned to the hospice program.

Position Qualifications

* Registered nurse with current active licensure to practice professional nursing in the State of CT.

* A Masters degree from a program approved by the National League for Nursing and or the American Public Health Association with a minimum of one year of full time clinical experience in a home health agency or related community health care program which include care of the sick or

* Graduate of NLN accredited school of nursing with a BSN degree and 3 years of full time clinical experience in nursing at least one of which was in a home health agency or community health program which included care of the sick at home or

* A diploma of nursing or an associates degree in nursing with minimum of three years of full time or full time equivalent clinical nursing experience in the last five years at least one year in a home health care agency or community health program which included care of the sick at home. Or

* Evidence of certification by the American Nurses Association as a community health nurse or completion of at least six credits in health care management from an accredited college or university program or school of nursing.

* Ideal candidate will have previous management or supervisory experience. Proven ability to work within an interdisciplinary setting.

* Complies with accepted professional standards and practice.

* Demonstrates excellent observation, verbal and written communication skills.

* Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

Essential Job Functions/Responsibilities

Clinical Coordination

* Makes patient and personnel assignments, coordinates patient care, coordinates referrals, assures that the patient needs are continually assessed and assures the development and implementation and updates of the individualized plan of care.

* Assigns the caseload of patients in their area of responsibility. Assures that patient and family needs are continually assessed, prioritized, and met in order to achieve desired clinical outcomes.

* Uses the principles of Patient-centered, family-focused care to interact with patients, families, physicians, and other care providers to identify agency strengths and weaknesses and intervene as necessary to provide for achieving desired patient outcomes. Assures the development, implementation, and updates of the individualized plan of care for patient.

* Assures that all Agency staff members deliver respectful care consistent with the patient/familys home, cultural, and environmental circumstances.

* Collaborates with appropriate team members to develop and implement patient specific plans of care.

* Promotes the development of care and practice standards and clinical pathways using benchmark/best practice markers and current research findings.

* Assists with the establishment, monitoring, and maintenance of the clinical documentation system that makes evident patient/family needs, interventions, and progress in accordance with clinical, quality, and regulatory requirements. Assures that all required documentation is available in the electronic record as per agency policy.

* Observes and promotes patient and family involvement in care planning, team conferences, and other mechanisms.

* Conducts regular team conferences with other home care disciplines to promote information exchange, determine care assignments, and to promote integration of care plan.

* Participates in field visits and conducts point of care (POC) supervisory visits to supervise staff in the area of responsibility. POC visits should be conducted according to the Agency Policy on Field Competencies and occur at least twice annually for all field staff. May also visit customers to follow-up on risk management or satisfaction issues.

* Monitors ongoing patient care, through care conferencing and review of documentation, to determine that educational and discharge planning needs meet the requirements of the CoPs and are planned and implemented to ensure the highest quality care for all HHA patients at all times.

* Is responsible to ensure written plan of care is provided to patients that includes the schedule visit frequency of Agency and contract staff, dosing frequency and meds administered by staff, treatments and other pertinent instruction, and the name and contact number of the Clinical Manager .

* May generate HHA assignments and do the supervisions.

* Ensures that there is ongoing clinical coordination and education for the patient and caregiver including discharge information.

* Values and incorporates the results of, and techniques/principles associated with, scientific inquiry and quality management in professional practice.

* Assists with the development, analysis, and creation of clinical improvement opportunities through the use of quality and patient feedback systems. Receives and works to resolve patient/family concerns/complaints.

* Maintains Clinical Records, statistics, and reports needed for purposes of evaluation and reporting

* The Supervisor of Clinical Services - Clinical Manager is an important member of the QAPI committee that meets quarterly

Service Management

* Assures compliance with all local, state, and federal laws regarding licensure and certification of agency personnel and maintains compliance to Home Care standards. Knows and holds staff accountable for care based on contractual and reimbursement regulations/requirements.

* Develops, knows, interprets, and implements policies, procedures, safety practices, and emergency plans for the safe and effective delivery of home health care services provided.

* Ensures referrals for all related disciplines are made as indicated by comprehensive assessment and patient condition.
