Duties:
- Provide home-based and remote case management to families with children ages 0-3 with developmental delays.
- Collaborate with the family to develop the Individualized Family Service Plan and outcomes for family supports, strategies and relevant services to be provided by our multi-disciplinary team of therapists and educators.
- Conduct initial and ongoing assessment of the child's development and functional skills, review progress, and outcomes to determine the progress made by each client while receiving early intervention services.
- Identify and refer families to community resources that support IFSP outcomes, address immediate family needs, or support natural learning opportunities.
- Ensure that all events and documentation are completed within the program's required timelines.
- Bachelor's Degree in Early Childhood Education, Child Development; Early Childhood Special Education, Social Work, Psychology, Communication Disorders, Nutrition or a related EI field.
- Bachelor's Degree in a non-related field and 3+ years' experience working with or caring for infants and toddlers with special health care needs and their families.
- Or a High School diploma and 6+ years' relevant experience with approval from EOHHS.
