Social Worker MSW - PACE/Elderplace

This position will require traveling to see patients in Washington county area.

Schedule: 40 hours/week, working weekday Day shift

Job Description:

The CSWA Social Worker, under direct and continuous supervision, provides comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems. The CSWA incorporates concepts and theories associated with life span development, neurobiology, and behavioral change. The CSWA will collaborate with the patient and support system(s) to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter/Multi-Disciplinary Team (IDT). The CSWA shall perform duties in a manner that respects patient self-determination that reduces health disparities/barriers to care and adheres to the NASW code of ethics and Standards of Practice (SOP)

Required qualifications:
  • Master's Degree in Social Worker.
  • upon hire: Oregon Clinical Social Work Associate License. Or
  • upon hire: Oregon Clinical Social Worker License.
Preferred qualifications:
  • 2 years Clinical work experience within healthcare, hospice and/or bereavement.

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life. Our service lines include assisted living/skilled nursing/rehabilitation, home health, home infusion/pharmacy services, home medical equipment, hospice and palliative care, Program of All-Inclusive Care for the Elderly (PACE), personal home services/private duty care and supportive housing.
