Key Responsibilities
  • Assessment and Intervention: Conduct thorough assessments of patients' psychosocial needs, develop and implement individualized plans to address these needs, and provide support to patients and their families.
  • Discharge Planning: Collaborate with the healthcare team to develop and implement discharge plans that meet patients' needs and ensure a smooth transition to post-acute care.
  • Community Resource Referral: Identify and connect patients with community resources that can provide support and services to meet their needs.
  • Documentation and Communication: Maintain accurate and timely documentation of patient interactions, assessments, and interventions, and communicate effectively with the healthcare team to ensure seamless care coordination.
  • Professional Development: Stay up-to-date with best practices in social work and healthcare, and participate in ongoing education and training to enhance skills and knowledge.
Requirements
  • Education: Master's degree in social work (MSW) from an accredited program.
  • Experience: At least two years of experience in a healthcare setting, with a focus on social work or a related field.
  • Licensure: Clinical Social Work Associate (CSWA) required upon hire, with the ability to obtain full licensure as a Licensed Clinical Social Worker (LCSW) within three years.
  • Skills: Excellent communication and interpersonal skills, ability to work effectively in a fast-paced environment, strong assessment and intervention skills, and proficiency in electronic medical records (EMRs).
Preferred Qualifications
  • Bilingual Skills: Bilingual skills are a plus, but not required.
