**Responsibilities**
This position serves as a liaison between the Oncology Nurse Navigator, Physician care team, hospital ancillary departments and referral sources for patients who are referred for oncology or benign condition services. This position works in partnership with the nurse navigator to expedite the referral and evaluation process for potential oncology patients and/or coordinates important care appointments. This position also provides organizational support for oncology patients by ensuring receipt of test results, facilitating correspondence, appropriately triaging patient phone calls and maintaining records.
**Essential Key Job Responsibilities**
1. Coordinates the patient evaluation process for oncology patients by facilitating referrals, insurance verification, obtaining test results, scheduling appointments and ensuring timely communication is provided to all team members including the patients during the entire process.
2. Identifies patients who are eligible for care and who may need a navigators services.
3. Able to assess needs by asking patients questions to understand their needs and what may be preventing them from care and then further working to overcome those barriers.
4. Initiates and follows through on pre-certification requirements, referrals and communication with patient's insurance company/PCP.
5. Coordinates care through making appointments, helping patients get to them, and coordinating care between different healthcare providers.
6. Organizes and maintains patient charts, obtaining correspondence and test results, and sorting results which need physician review prior to accessing care.
7. Obtains and communicates clinical information from patients, families, medical record and other members of the health care team to nurse navigators and providers.
8. Completes electronic documentation when providing navigation services, transitioning care, or ending navigation as well as the charge process associated with navigation point of service.
9. Provides financial aid application for patients needing medication and medical care assistance.
10. Demonstrates accountability for patient safety and reporting concerns in an accurate and timely manner to members of the team.
11. Coordinates patient clinic experience and assists in facilitation of actual clinic visit.
12. Offers emotional and educational support as appropriate to patients and their families.
**Qualifications**
**Required Education and Experience:**
+ High School Diploma/GED
+ Two years of relevant experience
**Required Minimum Knowledge, Skills, Abilities and Training:**
+ Computer skills required
+ Experience in a health care setting required and familiar with medical terminology
+ Requires excellent communication skills with members of the health care team, community facilities and vendors, and most important, patients and their family members
+ Demonstrates ability at self-direction, patient advocacy, follow through, and evaluation of services rendered.

**Pay Range**
$21.70 - $29.84 /hour
