Requirements and Qualifications:Employee must be bilingual.Bachelor's degree in early childhood education, Social Work, Nursing, or a related field is recommended. A high school diploma or GED and experience working with children and families accepted.Must pass criminal background check and is subject to the agency's drug and alcohol testing program.Negative Tuberculosis Screen, First Aid certification, CPR certification.Extensive travel required in the area; must be able to travel outside the project area as assigned.May come into contact with childhood diseases and blood on occasion.Possess a valid Texas driver's license and acceptable driving record, as approved by the agency's vehicle insurance carrier.Employee must have an account with a financial institution that can process financial transactions through MET's electronic payroll system.Computer literacy in Microsoft Office Suite and basic computer functions.Job Duties:Identify and recruit eligible HOPES families including children with disabilities and pregnant women.Establish a comfortable working relationship with families beginning at time of enrollment.Assist individuals with applications for Community Health including explaining information accurately and in understandable terms, interviewing families to obtain information needed for application, and facilitating computer-based enrollments.Ensure proper documentation and data collection as needed for program reporting and tracking.Establish working relationships with appropriate community partners to help advocate for the families in the community. Ensure MET has updated, working MOUs with all community partners.Provide education, information, resources, and support related to mental health and social services (housing, employment support, parenting education) and basic assistance.Provide rapid response immediate referrals and education to families that are in crisis using assessments, motivational interviewing, and through the dissemination of resources.Keep adequate documentation of parent contact and follow-up.Participate in and/or attend staff and parent meetings, as directed by supervisors.Perform CPR and First Aid administration as a collateral duty.Maintain confidentiality in all areas of responsibility.Report data into PEIRS.Drive the Wellness Wagon and maintain vehicle checks and report to supervisor.Perform all other duties as outlined by supervisor.
