Responsible for performing extended social work program development, case management, clinical research, outreach, patient assessment and evaluation requiring advanced skills and training.

Responsibilities

  • Performs psycho-social assessments, health education, supportive counseling and referrals; provides education on social services and public systems; assists patients in finding resources.
  • Interprets and assists in resolving social problems related to medical conditions and/or hospitalizations.
  • Collaborates with multidisciplinary team to provide comprehensive care; documents all patient/provider communication and interventions.
  • Coordinates clinical case management services including referrals for housing, meal assistance, medical transportation, and other needs as identified; formulates candid and supportive communication.
  • Coordinates ongoing counseling and support groups; facilitates effective problem solving and conflict resolution.
  • Assists with program evaluation activities including but not limited to data collection for grants, grant reporting, patient satisfaction surveys, and other activities as needed.
  • Provides Case Management services to support patients and families with Barriers to Care/Social Determinant of Health.
  • Performs all other duties as assigned.
Qualifications
  • Ability to independently conduct psycho-social assessments and provide treatment to a wide variety of individuals from various socioeconomic, cultural, ethnic, educational and other diverse backgrounds.
  • Knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures .
  • Independent judgment and skill in utilizing supportive, problem solving or crisis intervention techniques.
  • Experience using MS Office (Word, Excel, etc.) and data management software.
  • Excellent communication skills, both verbally and written; bilingual (Spanish/English) preferred.
  • Ability to maintain strict protocols of all confidential or sensitive information.
  • Ability to co-manage patients as part of a multidisciplinary team
EDUCATION:
  • Bachelors degree in Social Work is required.
LICENSE AND CERTIFICATION:
  • Licensed as a Social Worker in Texas (LMSW, LBSW, LSW).
