System One has partnered with a local organization in their search for a Client Care Coordinator to join their team of highly motivated individuals who have a passion for making a difference in their community. The Client Care Coordinator plays a pivotal role in the organization and is dedicated to empowering individuals within the community. This position involves comprehensive service provision, from assessing client needs to fostering independence through the coordination of community resources.

Essential Duties and Responsibilities:

  • Conduct Care Coordination services, including telephonic client assessments, eligibility determination, and response to condition changes.
  • Facilitate connections to community resources and monitor service delivery.
  • Make referrals, track issue resolution, and develop as well as maintain community referral partnerships.
  • Enhance operational efficiency and support the mission's growth through effective communication.
  • Maintain accurate records in various digital platforms, adhering to confidentiality and organizational standards.
  • Provide professional, compassionate interactions.
  • Perform additional duties as needed to further the mission of the organization.
Minimum Requirements:
  • Education & Experience: Bachelor's Degree (preferred) with a minimum of 2 years in care coordination or case management.
  • Legal: Must be a US Citizen or authorized to work in the US, and at least 18 years of age.
Skills & Abilities:
  • Communication: Exceptional oral and written communication skills.
  • Orientation: Goal-driven with a strong focus on the mission.
  • Professionalism: Capable of communicating effectively in a professional context.
  • Technology: Proficient in various software, including MS Office/Office 365.
  • Leadership: Experience in training and supervising team members.
