In our time of growth, MCR Health is seeking an OB Care Coordinator Navigator SMMT

Work Location(s): Bradenton, FL (Onsite)

As part of this role, you will:
  • Provide emotional support and education to high-risk pregnant women to help them understand the healthcare system, the stages of pregnancy, and their options for prenatal care
  • Enrolment within SMMT and distribution of program medical equipment such as scales, blood sugar monitors and blood pressure cuffs.
  • Facilitate communication and collaboration between healthcare providers, including obstetricians, nurses, midwives, and other specialists, to ensure seamless care
  • Help patients access resources, such as insurance, government assistance programs, and community services, to remove barriers to care
  • Offer educational resources to pregnant women on topics like prenatal nutrition, childbirth preparation, breastfeeding, and infant care
  • Collaborate with SMMT Program Coordinator and staff to assign cases to caseloads
  • Keep track of the patient's progress throughout pregnancy, ensuring they attend all necessary appointments, screenings, and tests
  • Maintain accurate records of patient interactions, care plans, and outcomes, while ensuring patient confidentiality
  • Be prepared to handle emergencies and provide immediate assistance in case of pregnancy-related complications
Requirements

What you need to bring to this role:
  • High School diploma Associate degree in a health care or public health field preferred
  • Medical Assistant or LPN preferred
  • 1-3 years' experience in a healthcare or public health area with direct patient interaction required
  • Knowledge of Medical Terminology preferred
  • Valid and unrestricted driver's license
  • Bilingual in Spanish a plus
