Your Role as a Hospice RN Case Manager As an RN Case Manager, you will play a critical role in providing exceptional home health and hospice care. Your responsibilities will include: Comprehensive Patient Assessments - Evaluating the physical, social, and psychological needs of patients. Collaborative Care Planning - Engaging patients, families, and interdisciplinary teams to develop holistic care plans. Patient-Centered Approach - Providing compassionate support to enhance comfort and quality of life. Resource Coordination - Managing patient care resources and referrals efficiently. Accurate Documentation - Maintaining meticulous patient records to ensure seamless care.
