_Home Health R N Case Manager _ HIRING BONUS RN CASE MANAGER The RN Case Manager/RN coordinates the care and services of adult patients in their homes. Duties include patient cares/visits and assessments, obtaining physicians orders and completing patient re-certifications, teaching, patient advocacy, supervion of nurses and home health aide services. Hours are Monday thru Friday and on call rotation. Great work environment. Flexibility with hours. Must have excellent communication and organizational skills .
