Your Role as a Hospice RN Case Manager As an RN Case Manager , you will play a critical role in providing exceptional home health and hospice care . Your responsibilities will include: ✔ Comprehensive Patient Assessments - Evaluating the physical, social, and psychological needs of patients. ✔ Collaborative Care Planning - Engaging patients, families, and interdisciplinary teams to develop holistic care plans. ✔ Patient-Centered Approach - Providing compassionate support to enhance comfort and quality of life . ✔ Resource Coordination - Managing patient care resources and referrals efficiently. ✔ Accurate Documentation - Maintaining meticulous patient records to ensure seamless care.
