Key Responsibilities: Conducts evaluation, treatment, consultation and discharge planning for children within the outpatient clinic and/or early intervention programs Use evidence-based interventions appropriate for each child and their written outcomes Documentation within the practice’s electronic medical record Maintain communication with child’s primary physician and other healthcare providers through progress reports, phone calls or other forms of communication Assess and make recommendations regarding appropriate assistive technology/adaptive equipment as needed, provide written justification, and coordinate purchase of equipment for a child with the family, team members, service coordinator, and other allied professionals.
