Provide comprehensive clinical care for patients with multiple comorbidities including CKD primarily via telehealth with occasional in-home visits. Utilize data analytics and performance metrics to track and monitor key quality and cost indicators, identify areas for improvement, and drive continuous quality improvement. Treat patients as the direct provider in collaboration with APPs and interdisciplinary team, providing evidence-based care including complex care through pharmacological and non-pharmacological interventions. Lead the development and implementation of clinical guidelines, protocols, and best practices to standardize care delivery and promote evidence-based medicine. Collaborate with executive leadership, clinical teams, and external stakeholders to develop and implement value-based care strategies that improve patient outcomes, reduce costs, and enhance the overall quality of care. Position Requirements

Medical degree from an accredited medical school. Board certification in Internal medicine, geriatrics, or family medicine. Active medical license and DEA registration in at least one state and willing to get licensed in multiple states in the region. Minimum of 3 years of clinical practice experience, with demonstrated leadership and management experience in healthcare delivery or administration. Excellent analytical skills, with the ability to interpret data, analyze trends, and make data-driven decisions. Exceptional communication and interpersonal skills, with the ability to effectively engage and influence stakeholders at all levels of the organization. Demonstrated ability to lead change, drive innovation, and implement complex healthcare initiatives in a dynamic and fast-paced environment. About Monogram Health
