Summary: Assists in the provision of care navigation and clinical activities. Assists the Wellness Team with access and utilization management, proactive patient panel management, and care facilitation. Prepares reports and uploads data extracts to/from existing operational systems and contract modeling software to support projects, medical cost assessments, and ongoing payor performance monitoring. Qualifications: Required:HS Diploma or passage of a high school equivalency exam.6 months of relevant patient care experience.Meets one of the following within the last 18 months:• Completion of an accredited Medical Assistant program and work experience in a Medical Assisting or acute patient care capacity, which includes school-sponsored externships.• Completion of a Medical Assisting-comparable military educational program for medics/hospital corpsmen with an honorable discharge (if applicable).BLS Certification from the AHA or obtained within 90 days of hire.Able to perform data entry.Able to use computerized word processing software.Familiarity with spreadsheets, databases, and patient registries.Knowledge of basic medical procedures and medical terminology.Solid written, verbal, and interpersonal communication skills.Able to collaborate with the health care team and diverse patient populations.Good judgment, flexibility, initiative, critical thinking, and problem-solving skills.Able to prioritize and organize tasks with attention to detail.Reliable transportation to make home visits to patients as needed.Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds. Preferred:Graduate of an accredited Medical Assistant program and completion of a school-sponsored externship.Certified Medical Assistant or Registered Medical Assistant.Registered in good standing as a State Tested Nursing Assistant (STNA) with the Ohio Department of Health Nurse Aide Registry.Fluency in writing, speaking, and interpreting both English and another designated foreign language.Knowledge of Epic electronic health record system.
