Job Summary

Responsibilities

  • Collaborate with case managers, physicians, and other healthcare professionals to identify patient needs and develop effective care plans.
  • Coordinate out-of-home placements, including referrals to community resources and social services.
  • Provide education and support to patients and families on healthcare services, community resources, and government entitlement programs.
  • Conduct psycho-social assessments and develop treatment plans to address patient needs.
  • Respond to complex patient issues, including abuse and neglect, and provide crisis intervention and support.
  • Participate in hospital committees and quality improvement initiatives to enhance patient care and services.
  • Document patient interactions and care plans in medical records.

Knowledge, Skills & Abilities

The ideal candidate will possess a Master's degree in Social Work or a related field and be licensed as a Social Worker. They will also have excellent communication and interpersonal skills, with the ability to work effectively with diverse patient populations and healthcare teams.
