Manatee Health at Home by BAYADA is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team as Transitional Care Nurse. As a Transitional Care Nurse, you will interact with discharge planners, case managers, and clients and families to coordinate the initiation of home health care services as well as treat patients in the comfort of their home. Qualifications:A current license as a Licensed Practice Nurse in the state of FL.A minimum of one year of recent, verifiable experience as an LPN.Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.Prior Home Health experience required.
