Join our team of competent and compassionate professionals as a shift Access Navigator in the CBHC at Behavioral Health Network

TheAccess Navigatorhas a unique role in team-based client care. You will build working relationships and provide in-the-moment interventions and referrals to support individuals. As an Access Navigator, you'll provide evidence-based care in the context of an integrated behavioral health team led by a Master's-Level Clinicians. Access Navigators serve as advocates, providing needs assessment, case management, direct telehealth and/or in-person support, and help to access needed services. This position requires understanding of community resources and providers, both internal and external to BHN.

What are we looking for in an Access Navigator?
  • Bilingual/bicultural candidates encouraged to apply
  • High School diploma with 1-2 years of work experience in related health care field (required).
  • Associate degree with 1-2 years of work experience in related behavioral health field (preferred).
  • Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management (required).
  • Professional demeanor and phone skills in communicating with clients and care team clients (required).
  • Familiarity with electronic medical record (EMR,) internet, word processing and email systems (preferred).
