Join Lutheran Social Services of the Southwest (LSS-SW), a nonprofit organization established in 1970 to assist our most vulnerable community members. LSS-SW is dedicated to delivering services that:STABILIZE during crises,BUILD self-reliant foundations, andPRESERVE dignity and respect.Each month, LSS-SW supports over 5,000 Arizonans across Metro Phoenix, Tucson, and southern Arizona through programs in Refugee Services, Aging & Disability Services, Family Services, and Emergency Services.Position Overview: Reporting to the Home Care Supervisor, the Care Coordinator provides in administrative support and program coordination in the provision of Home Care Services, including implantation of objectives and coordinating and monitoring Home Care Services. This position will work with all levels of staff throughout the department as well as community partners and funders. A high level of professionalism, sensitivity to the program population's cultural and socioeconomic characteristics is expected.Key Responsibilities:Daily Support of Home Care: Provide administrative support, ensure procedures are followed and deadlines are met. Maintain tracking and software systems to capture data and schedule authorized services. Identify and record information accurately to assist Clients, Staff Members, Case Managers, and the Home Care Supervisor. Be able to provide backup coverage in the event of an absence to conduct home visit intakes and reassessments at a client's home.Training and Development: Train and develop staff according to policy and procedures. Assist the Home Care supervisor in coordinating continuing education, community awareness trainings and workshops.Community Integration: Develop and maintain relationships with collaborating agencies, local communities and congregations. Represent the agency in public relations, educational and professional situations. Effectively present and represent LSS- SW and the program's needs to religious and community groups as well as others in the social services community, to include funders.Policy & Program Management and Communication: Working with the Home Care supervisor, participate in developing and implementing policies and procedures for the program, as well as monitoring staff to ensure compliance within them.Qualifications:Associate's Degree or two to four years related social service experience and/or training; or equivalent combination of education and experienceBi-lingual English/Spanish both written and verbal is strongly preferredValid Arizona driver's license, reliable transportation, and ability to clear background checks.Ability to manage time effectively, work independently, and handle confidential information with discretion.Proficient with computers and related business electronics, systems and software, including Microsoft Office products.Professional communications in all forms, strong customer service skills, effective listening, ability to ascertain needs and exercise good judgement.AHCCCS and DES approved Direct Care Worker Instructor (Training is available upon hire)Ability to sit or stand for extended periods, operate office equipment, and occasionally lift up to 50 pounds.Ability to work within the mission of LSS-SWJoin Our Mission:If you are passionate about making a difference and have a strong administrative support and program coordination background, join us in fulfilling the mission of LSS-SW to support and empower our community's most vulnerable members, and our vision of "A future where all people thrive."All positions at Lutheran Social Services of the Southwest require a background check.
