Working closely with the Nurse Care Manager, the Care Coordinator will be responsible for conducting daily patient follow-ups, processing durable medical equipment (DME) and home health orders, triaging calls, retrieving hospital records for recently admitted patients, and performing other clinical clerical tasks within the scope of practice for high-risk patients. Additionally, the Care Coordinator will undertake other tasks as assigned by the Nurse Care Manager.ResponsibilitiesConduct daily patient follow-ups for high-risk patients within the Chronic Care ProgramProcess DME and home health orders efficiently and accuratelyReceive inbound triage calls from patients and coordinate appropriate responses to acute patient needsRetrieve hospital records for patients recently admitted to external facilities Perform clinical clerical tasks to support the Chronic Care Program and Transition of Care ProgramsAssist in procedures within the Medical Assistant scope of practice for high-risk patients Collaborate closely with the Nurse Care Manager to ensure seamless patient care deliveryIdentify and address barriers to care for high-risk patients Coordinate patient care progression throughout the continuum, including transitions from acute and post-acute settings to home or other transitional care facilities Communicate effectively with physicians, nursing staff, and other members of the multidisciplinary care team Facilitate patient discharge planning process to optimize outcomes and satisfactionMonitor patient progress and intervene as necessary to ensure patient-focused, high-quality careCollaborate with external case managers and community resources as neededActively participate in clinical performance improvement activities Support activities to promote closure of care gaps and attainment of Medicare HEDIS metricsOther tasks as assignedWhat You'll BringKnowledge, Skills, and AbilitiesMinimum 2 years of experience as a Medical Assistant or a, LPN / LVNExperience in chronic care management or related field preferredExcellent interpersonal and communication skillsStrong organizational and time management abilitiesProficiency in Microsoft Office suiteBilingual/Bicultural (English and Spanish) requiredEducation, Experience, Licensure, or Certification RequirementsHigh school diploma or equivalentCompletion of a Medical Assistant program requiredCertification in Medical Assisting from AAMA, CCMA or any other nationally recognized body requiredLPN / LVN license preferredHow We WorkOur Culture & Core Beliefs Earn Trust Building Relationships Creating Joy Doing Right Improving Every DayMoving ForwardEqual Employment Opportunity (EEO) PolicySuvida Healthcare provides equal employment opportunities to all Team Members and applicants for employment and prohibits discrimination and harassment of any type with regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
