Proctor Place is a life-care retirement community located in Peoria, Illinois. At Proctor Place, we believe in providing our residents with the highest level of care possible by maintaining a focus on compassion and making a difference in the lives of our residents.

Proctor Place is looking for a LPN or RN to be our new Full-Time Care Plan Coordinator to join our Healthcare leadership team This position would be 1st shift, Monday-Friday, 8:00am-4:3pm. No weekends or Holidays. On-Call Rotation would be mandatory which will be every 4th weekend.

Pay will be $34.00-$36.00/hour for LPNs or $40.00-$42.00/hour for RN's

Job Purpose:

The main job purpose of this position is to assess resident's physical & mental function and document data on care plan forms completely and accurately and assist in addressing staff issues when care plan related.

Some of the Job Duties Consist of:
  • Communicating & collaborating with all disciplines in the resident assessment & care planning process.
  • Lead care plan meetings & family conferences
  • Establish appropriate level of care status for residents upon admission; providing quarterly updates and changes
  • Facilitate development of a comprehensive plan of care for each resident
  • Reviewing medical records related to resident's health to collect information related to the resident assessment
  • Provide direct nursing care as needed
  • Provide leadership to staff and mange adequate staffing levels
  • Perform other such duties as may be assigned
Knowledge, Skills, & Abilities:
  • Must possess the ability to seek out new methods & principles and be willing to incorporate them into existing nursing practices
  • Must function independently, have flexibility, personal integrity, & the ability to work effectively with residents and personnel
  • Must be able to handle information with sensitivity & confidentiality
  • Demonstrates positive attitude, problem solving abilities with attention to detail
Education/Experience:
  • Registered Nurse or Licensed Practical Nurse with a valid Illinois State license.
  • 3-5 years' nursing experience, geriatric setting preferred
  • Minimum of 1 year of leadership experience preferred
Physical Requirements:
  • Must be able to sit, stand, bend, lift and move intermittently during work hours
  • Must be able to life 50 pounds occasionally
  • Must be able to life, push, pull, and move equipment, supplies, etc, throughout the workday
