Transitional Specialist

NFI Massachusetts Inc., a non-profit human services agency, is currently seeking a Transitional Specialist for our Intensive Residential Treatment Program located in Westborough, MA.
The IRTP's are short-term locked facilities serving male and female youth who are referred through the Department of Mental Health (DMH). NFIs IRTPs are state-wide residential programs for adolescents who have a serious mental illness and/or severe emotional and/or behavioral disturbances.
The Transitional Specialist is responsible for connecting clients to services offered in the program and in the community which meet the needs of the consumers.
Hourly: $23.00/hr
Job Responsibilities:
- Find providers in the community for client post-discharge
- Act as a liaison between client and outside service providers (such as group homes and potential school placements) to coordinate visits to these agencies when a client is preparing for discharge
- Engage client in the community through immersive experience
- Assist client with independent living skills such as obtaining a license or applying for disability
- Follows along with the client for 3 months post-discharge to provide assistance to the client and collaborate with client's providers when needed. This includes visiting client in the community and maintaining contact with parents when necessary.
- Bachelors degree (B.A.) from four-year college or university in social behavioral science; and/or one to two years related experience and/or training; or equivalent combination or education and experience.
- High energy level, superior interpersonal skills and ability to function in a team atmosphere.
- Valid driver's license and a reliable vehicle is a must.
- Must be fully vaccinated, or willing to get vaccinated against COVID 19 per state regulations
