DescriptionThe Children's Home is seeking a Full-Time Coordinator of Care Management. This position is a member of the Care Coordination Team and will assist in the coordination and organization of all day to day functions in the hospital, daily maintenance of all medical records, and responsible for efficient and accurate scheduling of in-patient and outpatient services through the various programs of The Children's Home.
