ESSENTIAL FUNCTIONS AND BASIC DUTIES Coordinates programs and activities for eligible individuals on a group basis. Serves as a liaison to community agencies, networks with community service providers, and seeks out new services available to eligible individuals. Monitors the ongoing provision of services from community agencies and documents participant’s use in the designated documentation management system. Develops and maintains a Resource Directory of state and/or local service providers. Organizes educational events related to health care, agency support, life skills, and referral sources. Identifies partners to provide health services and screenings to participants. Assists participants in building informal support networks. Engages volunteers within the community and/or property when appropriate. Coordinates services for participants on an individual basis. Outreaches to eligible individuals to provide education about the program and offer enrollment. Conducts telephonic and/or in-person participant assessments and screenings. Collaborates and communicates appropriate information with care and service colleagues. Provides quality customer service to all eligible individuals. Assists participants in acquiring desired community services. Adheres to a follow-up and monitoring schedule. Performs related duties as required or assigned. Performs miscellaneous tasks as needed. Builds rapport with participants while maintaining professional boundaries. Manages time independently and completes required tasks and documentation. Responds to all communications in a timely and professional manner. Accurately records labor time according to policy. Documentation Completes timely electronic and written documentation. Completes all required forms with participants. Completes Care Plans and monitors plans with participants. Submits documentation for funder and quality Performance Reports. PERFORMANCE MEASUREMENTS Meets requirements outlined in Quality Assurance Review Process and other regulatory requirements. All responsibilities are carried out in a manner consistent with National Church Residence’s Core Values. EXPECTATIONS Maintains a caseload ratio of more than 1.25 assigned units/individuals per scheduled hours. Demonstrates and maintains a positive, professional manner. Adheres to all policies and procedures set forth in the Employee Information Guide. Ensures all care is provided with respect and dignity. Completes all annual education requirements timely. QUALIFICATIONS EDUCATION/CERTIFICATION: High School Diploma or GED required; a bachelor’s degree in social work or a related field is preferred. EXPERIENCE REQUIRED: Two or more years in social service delivery with elderly or disabled populations. SKILLS/ABILITIES: Good communication, comprehension, and interpersonal skills. Working knowledge of Windows, Microsoft Office Suite, and World Wide Web. PHYSICAL ACTIVITIES AND REQUIREMENTS OF THIS POSITION Travel: Occasional or as directed. Vision: Normal, consistent with standard workflow. WORKING CONDITIONS Worker is subject to changing inside and/or outside temperatures. MENTAL ACTIVITIES AND REQUIREMENTS OF THIS POSITION REASONING ABILITY: Ability to apply common sense understanding to carry out detailed instructions.
