DescriptionThinks, communicates and behaves as a cause-driven leader and role model for other staff and members when it comes to desired staff behaviors; promotes youth development, healthy living, and social responsibility in all job-related functions.Contact families within 72 hours of referral.Describe Stable Families program requirements to determine program eligibility.If families meet program requirements (reside in Franklin County and have a school age child in public school) schedule initial visit to complete assessments and gather program information.Meet with families in their homes, at school, or YMCA locations, depending on the family's preference.Complete a an initial assessment with each family at program entry as part of the initial phone call or face to face home visit to determine family priorities.Conduct at minimum, two face-to-face meetings per month with each family for the first six months of program enrollment. Conduct one face-to-face and one phone meeting per month the second six months.Gather income information from family, TANF eligibility, FCDJFS benefits, income from SSI/D, and/or employment. Develop a Family Stabilization Goal Plan within seven days of initial intake to create strategies for the family to stabilize and thrive. Update the plan every three months as goals are met and develop new goals with the family. As part of the initial assessments link families with resources to ensure wrap around support and successful implementation of goals. Utilize the Siemer Institute Family Assessment tool and Arizona Self-Sufficiency Matrix with each family at program entry and exit. Promote and utilize the two-generation (2-gen) approach to meet the needs of children and adults simultaneously to foster academic success and break the cycle of poverty.Engage and complete motivational interviewing with families for program enrollment and complete a thorough intake process to determine family strengths and needs. Act as a resource to families and provide appropriate linkage and referralAssist families by providing budget coaching, financial literacy, and job training opportunities.Leverage community resources and refer families to other community programs. Administer family assessments, case plans, resource coordination and financial literacy and education for all families on caseload.Enter and/or turn in data within 48 hours of activity.Complete an ongoing employment tracker with each family and monitor employment progress as part of ongoing assessment.Develop positive relationships with landlords and rental facilities in order to explore housing avenues for families. Monitor academic progress and school attendance of children in the program. Connect families and children with appropriate resources in order to maintain and increase attendance and grades for children in the program.Support families so they may maintain and increase their child's academic success.Develop relationships with elementary, middle, and high schools in order to ensure each child has the supports and services they need at school. Carry a caseload of at least 50 open cases/families at any given time, serve approximately 75 families in each 12 month program cycle, communicate with the families with level of required frequency, and monitor ongoing progress.Maintain clear and effective case notes and files for all families receiving services and maintain required documentation.Meet one-on-one with each family in order to develop a relationship in order to guide them out of crisis and into stability so they are empowered to meet their stabilization goals. Attend training on motivational interviewing, community resources, trauma informed care, and cultural competency.Collaborate with program partners in schools, community organizations, and other YMCA leaders and programs.Complete an informal assessment every three months to monitor family income levels and housing stability. Work intensely with families for approximately 6-12 months and complete exits when family stabilization levels have been met, as measured by the Siemer Family Assessment, Arizona Self-Sufficiency Matrix, and supervisor.RequirementsQUALIFICATIONS:Bachelor's Degree (or equivalent combination of education and/or experience) in a social services field (social worker, psychology, child development, education, or related field), experience with home visiting and knowledge of housing and employment resources. Passionate belief in the Y's cause of nurturing the potential of all youth, supporting healthy living for all people and finding ways to help and support our neighbors.Ability to relate effectively to diverse groups of people from all social and economic segments of the community.Knowledge of word processing, spreadsheet and database programs.Must have good time management skills, have good attention to detail, and be competent in Windows/Excel applications.Proven track record of developing authentic relationships with others.Ability to foster a collaborative team approach to solving challenging situations. YMCA Team Leader certification; if not certified, must obtain certification within 12 months
