Job Summary The Case Manager works as part of the Support Team for Educational Partnership (STEP) behavior/climate team to provide quality case management services for all students in compliance with Medicaid regulations. The Case Manager will work within the school?s Multi-Tiered System of Support framework to address the social, emotional and social services needs of students and families. The Case Manager will also provide case management services to students and families to promote and spport students? physical and emotional wellness to bolster academic and social success. This position is developed in collaboration with Community Behavioral Health and the City of Phildelphia. Essential Functions Engages with the family and assists with identifying resources to enhance treatment; works collaboratively with STEP to assist with obtaining necessary outside supports for students. Oversees comprehensive educational support services to address the social, emotional and social service needs of students and families by addressing unmet physical, living situation and behavioral health needs to help stabilize the child and family. Provides case management services to students and families to promote and support students' physical and emotional wellness to bolster academic and social success; develops a plan of action and implements it to successful completion. Provides referral services and access to resources to support students and families; serves as the link between the home, school and community. Maintains and updates client case files and records by reviewing case notes, logging events and tracking progress. Assists in the development and implementation of appropriate programmatic and administrative policies; ensures compliance with STEP and School District of Philadelphia policies, and Medicaid regulations; follows all state Medicaid guidelines as it pertains to service quality and documentation. Makes home visits to encourage parents to provide support for students. Minimum Requirements Bachelor's degree from an accredited college or university in social work, social services, human services or a related field. Two years of full-time, paid, professional experience providing case management to children and families in an urban environment. All work experience related to the position must be included on the resume. Knowledge, Skills and Abilities Demonstrated knowledge of: the principles, practices and techniques of case management. the practices and tecniques of providing students and families with optimal customer service. school-based programs. Ability to: promote and identify resources available for the students and families. participate as an active member of STEP. ensure case records meet documentation standards. analyze problems and data with discretion. maintain effective working relationships with students and families. communicate effectively, both orally and in writing. establish and maintain effective working relationships.
