Responsibilities Michael's House is currently seeking a dynamic Patient Care Coordinator - full time. The Patient Care Coordinator is responsible for the coordination of administrative and discharge planning of patients.Do you have a passion for working with patients with co-occurring disorders and chemical dependence? If so, then you would be a wonderful addition to our team This position is located at UHS facility, Michael's House, in Palm Springs, California.Benefits include:•Challenging and rewarding work environment•Competitive Compensation & Generous Paid Time Off•Excellent Medical, Dental, Vision and Prescription Drug Plans•401(K) with company match and discounted stock plan•Education and Student Loan Reimbursement•Career development opportunities within UHS and its 300+ Subsidiaries Qualifications Essential Duties and Responsibilities•Assist the patient in coordinating immediate outside concerns such as legal obligations, employment and/or disability documentation.•Works with the patient and treatment team to develop a comprehensive aftercare plan.•Documents all interactions with patients within the electronic medical record•Provides communications/updates to the treatment team, families, and other referral resourcesEducation and Experience•Bachelor's degree in psychology, social work, or related field required.
