About the Job
As a Patient Care Coordinator RN at Singing River Health System, you will play a vital role in ensuring quality and cost-effective care for non-hospitalized patients. Your responsibilities will include coordinating interdisciplinary plans of care, liaising between patients and healthcare personnel, and promoting patient outcomes.
Key Responsibilities:
- Coordinate care for multi-visit patients, addressing social determinants of health.
- Ensure all necessary care is provided to patients.
- Promote quality and cost-effective care and patient outcomes.
Requirements:
- Graduate of an accredited School of Nursing.
- Currently licensed to practice in the State of Mississippi as a nurse.
- 2+ years direct patient care experience or clinic experience.
- Experience with Case Management preferred.
Work Environment:
The work environment for this position is moderately active, requiring frequent movement about the office and facility. You may be exposed to body fluids and tissues, which may carry the hazard of infectious disease. Ability to be active for extended periods without experiencing undue fatigue is essential.
Mental Demands:
- Keen mental faculties/assessment and decision-making abilities.
- Superior communication/speaking/enunciation skills.
- Strong written and verbal communication skills.
