CHIPA Director of Population Health
Overview
Reporting to the Senior Vice President of Population Health, the Director of Population Health oversees the Population Health team to support member FQHCs in their efforts to improve clinical quality measures, ensure the wellness of members, and meet clinical measures contract goals. The Director works closely with a variety of stakeholders, coordinating the activities of Population Health across the enterprise to develop, implement, and evaluate Population Health programs and initiatives.
They offer support to clinical workgroups and achieves improvement by identifying and advancing best practices and other innovations, and improvements that can be spread across the network. They carefully track quality data and work with stakeholders to maximize CHIPA's performance on all contracted quality metrics.

Responsibilities: Department Leadership and Stakeholder Collaboration:
•Provides direct oversight of CHIPA's Population Health Specialists
•Supervises and coaches staff as they support FQHC partners with improving their performance on clinical quality measures
•Enables staff to achieve optimal results by providing information, resources, encouragement and support
•With elected co-chairs, appoints Population Health Specialists to staff each clinical workgroup as needed. Seves as alternate to facilitate workgroup when needed.
•Leads and/or serves on a variety of appropriate committees and workgroups, including attending meetings with stakeholders (i.e., managed care plans, vendors)
•Reviews and analyzes network and health center specific member data related to clinical care and outcomes to evaluate performance, reveal trends, and identify opportunities. Shares findings with appropriate CHIPA leadership and practice to support decision-making efforts
•Remains knowledgeable regarding the most up to date measure specifications for each contract measure
•Demonstrates the highest level of professionalism, integrity and ethical values. Presents themselves in a positive disposition, is highly effective in communicating (written and oral) with staff and network partners to foster successful team collaboration, and encourage a positive organizational culture

Responsibilities: Practice Development and Quality Improvement
•Helps assigned practices adopt, transform, and sustain CHIPA's value-based model of care
•Acts as a principal contact for assigned partner FQHCs
•Establishes a strong and positive culture of collaboration with assigned CHIPA partner FQHCs, including but not limited to:
o Educating and training health center teams on quality measure specifications, including knowledge of appropriate billing codes and/or documentation needed by the payor to meet measure compliance
o Designing, implementing, and monitoring quality improvement processes, including continuous assessment of challenges in achieving stated goals
o Coordinating additional training initiatives needed by the FQHC, such as in the Population Health analytics tool and its application
o Assesses the capability of the FQHC to meet contract goals, considering its resources and experience around KPIs
•In collaboration with CHIPA partners, develop action plans and follow-up for each quality measure to maximize contract terms
•Recommends resources that may be helpful to FQHCs, such as access to community resources, and self-management materials in multiple languages
•Recommends changes in workflows and adoption of new technologies (i.e. texting platforms and retinal imaging camaras) at partner sites. Identifying new/needed skills, processes, or methods to support a culture of performance improvement (i.e. project management and change management training)
•Building and coordinating innovative partnerships with multiple stakeholders (i.e. sharing best practices among partners)
•When appropriate, research additional best practices to help with improvement initiatives. Coordinate meetings (in person or virtually) with health center and CHIPA staff for the purposes of training, and to discuss the performance of selected projects

Responsibilities: Facilitating Workgroup
•With selected chair and co-chair, facilitates a monthly peer-to-peer workgroup comprised of health center staff, and at times, consultants. This includes, but is not limited to coordinating meeting schedules, completing meeting notes and meeting materials, distributing such materials and notes to the workgroup, and maintaining a central repository for documents and information from past meetings
•Develops workgroup projects, objectives, goals, and roadmaps
•Encourages collaboration between workgroup members, as well as with other CHIPA existing workgroups
•With input from workgroup leaders, summarize and report on workgroup progress to the Clinical Strategy Committee
•When needed, assists in facilitating the Clinical Strategy Committee

Responsibilities: Data Analytics & Reporting
•Coordinates practice training on the data analytics platform and makes recommendations for additional staff training. Provides coaching to practices so they can independently access, analyze, and utilize analytic platforms and review their performance on KPIs.
•Assists in creating performance dashboards and facilitating the request of custom reports to ensure the success of partners
•Validates MCO and analytics platform data to ensure integrity of information and data hygiene using data mining techniques. Works with the appropriate liaison to achieve data accuracy.

General Duties:
•Manages assigned staff time and attendance, to include requests for Paid Time Off
•Oversees hiring, orientation, training and development of assigned team members
•Provides coaching and counseling to assigned staff utilizing approved performance feedback tools and methods
•Maintains a strong focus on ensuring that all work efforts are aligned with organizational goals and objectives
•Be familiar with and adhere to all administrative and general policies and procedures
•Maintain confidentiality of all aspects of CHIPA including, but not limited to member confidentiality, operations, financials, and employee relations
•Other duties as assigned

Education: High School Graduate/GED required

Salary: $100K-120K/ annually
