Description

PURPOSE: The Wellness Director is responsible for the overall implementation, delivery, and coordination of the resident care services at Loyalhanna Senior Suites and Personal Care. By empowering others and leading by example, exceptional standards of care developed and maintained.

Resident Rights
  • Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
  • Ensure that you treat all residents fairly, and with kindness, dignity, and respect
  • Ensure that all nursing is provided in privacy
  • Knock before entering the resident's room
  • Report all grievances and complaints made by the resident to the Administrator
  • Report all allegations of resident abuse and/or misappropriation of resident's property immediately to the Administrator
  • Honor the resident's refusal of treatments request. Report such requests to your supervisor
  • Communicate respectfully and appropriately, respect culture differences among residents and staff
ESSENTIAL RESPONSIBILITIES AND DUTIES:
  • Admit, transfer, and discharge residents as required
  • Perform referral assessments with SNF building for potential admissions
  • Complete and file required recordkeeping forms/charts upon the resident's admissions, transfer, and/or discharge in Point Click Care.
  • Direct the day-to-day functions of the pc aides/med-techs in accordance with current rules, regulations, and guidelines that govern the personal care facility
  • Participate in facility surveys (inspections) made by authorized government agencies as may be requested
  • Complete accident/incident reports as necessary
  • Perform administrative duties such as completing medical forms, reports, charting and etc.
  • Agree not to disclose assigned usernames and passwords for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Administrator
  • Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator
  • Report any known or suspected unauthorized attempt to access facility's information system
  • Complete and file required recordkeeping forms/charts upon the resident's admissions, transfer, and/or discharge in Point Click Care.
  • Receive telephone orders from physicians and record on the Physician's Order Form.
  • Transcribe physician's orders to resident charts, Kardex, medication cards, treatment/care plans, as required.
  • Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident's response to the care.
  • Fill out and complete accident/incident reports. Submit Director as required.
  • Chart all reports of accidents/incidents involving residents. Follow established procedures.
  • Record new/changed diet orders. Forward information to the food services department.
  • Fill out and complete transfer forms in accordance with established procedures.
  • Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures.
  • Sign and date all entries made in the resident's medical record in Point Click Care
  • Must attend and participate in continuing education programs designed to keep you abreast of changes in your profession, as well as maintain your license on a current status.
  • Attend and participate in outside training programs.
  • Attend and participate in monthly/annual facility in-service training programs as scheduled.
  • Report absentee call-ins to the Nurse Supervisor. Fill out call off form and return to the Scheduler/Human Resources.
  • Provide leadership to the nursing personnel assigned to your unit/shift.
  • Make daily rounds of your unit/shift to ensure that nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Report problems areas to the Nurse Supervisor.
  • Develop and maintain a good working rapport with the inter-departmental personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of residents.
  • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
  • Inform nursing personnel of new admissions, their expected time of arrival, room assignment and etc. All must be charted into Point Click Care.
  • Ensure that rooms are ready for new admissions.
  • Greet newly admitted residents upon admission. Escort them to their rooms as necessary.
  • Participate in the orientation of new residents/family members to the facility.
  • Consult with the resident's physician in providing in resident's care treatment, rehabilitation, etc., as necessary.
  • Review the resident's chart for specific treatments, medication orders, diets, etc.
  • Implement and maintain established nursing objectives and standards.
  • Notify the resident's attending physician when the resident is involved in an accident, incident, or change in condition.
  • Carry out restorative and rehabilitative programs, to include self-help and care.
  • Inspect the nursing services treatment areas daily to ensure that they are maintained in a clean and safe manner.
  • Administer professional services such as; catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead/dying, etc.
  • Obtain sputum, urine and other specimens for lab tests as ordered.
  • Take and record temperature, pulse, respirations, blood pressures and accuchecks.
  • Monitor seriously ill residents, as necessary.
  • Check food brought into the facility by resident's family/visitors to ensure that it is within the resident's dietary allowances.
  • Ensure that personnel providing direct care to residents are providing such care in accordance with the resident's care plan and wishes.
  • Ensure that residents who are unable to call for help are checked frequently.
  • Admit, transfer and residents, as necessary.
  • Assist in arranging transportation for discharged residents as necessary.
  • Ensure that residents are escorted to pick-up area.
  • Inform family members of the death of a resident, call the funeral homes when requested by the family. Ensure the established postmortem procedures are followed.
  • Establish and maintain a medication system which adheres to determine system which adheres to state, federal, and company regulations and guidelines.
  • Operate the Wellness Department within the budget and according to the Loyalhanna Policies, procedures, and standards.
  • Complete Quality Assurance reviews to ascertain if the department is meeting state regulations and Loyalhanna Policies.
  • Acts as a contact for issues related to the resident care within the community.
  • Provide training and orientation to the new personal care staff members and provide ongoing training to current staff members.
  • Builds and maintains productive, work-focused relationships with the community staff, residents, their families and within the local community.
Safety and Sanitation
  • Monitor your assigned personnel to ensure that they are following established safety regulations in the use of equipment and supplies.
  • Ensure that established departmental policies and procedures, including dress codes, are followed by your assigned nursing personnel.
  • Ensure that the adequate supply of personal protective equipment is on hand and are readily available to personnel who performs procedures that involve exposure to blood or body fluids.
  • Ensure that your unit's resident care rooms, treatment areas, etc., are maintained in a clean, safe, and sanitary manner.
  • Ensure that your assigned personnel follow established handwashing and hand hygiene technique in the administering of nursing care procedures.
  • Ensure that all personnel wear and/or use safety equipment and supplies (i.e. back brace, mechanical lifts, proper lifting technique, etc.) when lifting or moving residents.
  • Have the knowledge of what is in the MSDS binders, report missing/illegible labels and MSDSs to the safety officer or other designated person.
SPECIFIC REQUIREMENTS:
  • Must be able to read, write, speak, and understand the English language
  • Must be a supportive team member, contribute to and be an example of teamwork and the team concept
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public
  • Must be able to follow written and oral instructions
  • Must not pose a threat to the health or safety of their individuals in the workplace
  • Must be able to move intermittently throughout the workday
  • Must be able to function independently and have flexibility, personal integrity, and the ability to work effectively with other
  • Must be willing to perform tasks that may involve exposure to the blood and/or body fluids
  • Must be willing to assist in the evacuation of residents during emergency situations
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
  • Must perform all additional tasks assigned to you by supervisor
Qualifications

QUALIFICATIONS:
  • Current PA State Licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
  • Experience in direct patient care, and assessments/observation of clinical conditions.
  • Experience in recruiting, training, and managing staff in the healthcare field.
  • Experience at a department head level in Personal Care or a related field.
  • Demonstrated leadership with supervisory skill.
  • At least 3 years' experience in long term care/geriatrics.
  • Must possess excellent communication, organizational and observations skills
  • Ability to work well with others and communicate effectively
  • CPR certified and knowledge of basic first aid
  • Must completed all state required training to maintain licensure or certification
  • Working knowledge of the PA State regulations for Personal Care Home and experience in Personal Care is highly preferred.
  • Able to work flexible hours due to the demands of the position
  • Must possess valid driver's license
