Description

WESTSIDE IS LOOKING FOR A SOCIAL SERVICES TEAM MEMBER

JOIN THE TEAM THAT TAKES CARE OF YOUR COMMUNITY

The School Based Social Services Coordinator is a core member of the collaborative care team who provides access to social services and resources in the community. As a School Based Social Services Social Services Coordinator, you will be instrumental in creating a nurturing and supportive environment for students and their families at East Side Charter and in the surrounding Wilmington community. This dynamic role encompasses proactive engagement with parents and the school, close collaboration with local organizations, and the provision of direct support to address social and health-related concerns for students and their families. The ideal candidate will excel in working on a multi-disciplinary team, ensuring meticulous documentation, and serving as a pivotal link between the school system and the broader community.

WORKING AT WESTSIDE MEANS WORKING IN A PRIMARY CARE MEDICAL HOME.

A Primary Care Medical Home is not a special building. It is a way to provide healthcare that puts the patient at the center of health care decision-making. As a community health center, Westside Family Healthcare provides care for everyone who walks through our doors, regardless of ability to pay, immigration or citizenship status, national origin, religion, race, ethnicity, gender identity, or sexual orientation.

OUR MISSION: To improve the health of our communities by providing equal access to quality healthcare, regardless of ability to pay.

OUR BENEFITS: Our benefit package includes medical insurance, dental insurance, vision insurance, life and disability insurance, a 401(k) retirement plan with a match, and supplemental insurances. We offer a generous PTO package and flexibility to provide work/life balance.

What this role does:

  1. Establish and nurture relationships with EastSide families and staff through active participation in parent events, staff meetings, and community gatherings.
  2. Facilitate workshops, events, and informational sessions to promote health and wellness awareness.
  3. Collaborate with local organizations to enhance community support services in the Wilmington community where EastSide Charter families live.
  4. Provide direct support to students and their families, addressing social and health-related concerns.
  5. Perform comprehensive screenings and assessments of a patient's social, emotional, environmental, financial, and other support needs to identify factors influencing the student's well-being.
  6. Collaborate with the interdisciplinary team to create and implement individualized care plans.
  7. Assist families in navigating and accessing available community resources, social services, and healthcare facilities.
  8. Stay informed about local resources and maintain an up-to-date database for easy reference.
  9. Advocate for the social and emotional well-being of students and their families within the school system and the broader community.
  10. Work collaboratively with EastSide Charter staff to address social issues impacting student success.
  11. Occasionally support the operational needs of the School-Based Health Center that may include patient registration, schedule appointments, and other duties as needed.
  12. Maintain accurate and timely documentation in the electronic medical record. Document progress and treatment recommendations in EMR and the registry so that information is shared with medical providers, psychiatric consultants, and other treating providers.
  13. Perform other duties as assigned.
Requirements
  1. High School Diploma or General Equivalency Diploma (G.E.D.)
  2. Strong written and oral communication skills
  3. Self-starter with the capability to work independently and collaboratively.
  4. One year of experience providing services to a culturally diverse population
  5. Three years of experience using computers and Microsoft Office software
  6. Ability to perform duties directly in the communities as needed (up to 90% of the schedule will be required to work in communities throughout Delaware)
  7. Verifiable good driving record and reliable transportation
  8. Ability to work evenings and weekends as needed.
