Patient Clinical Medical Navigator/ Staff RN

Job ResponsibilitiesCollaborating with multi-disciplinary departments and providing expertise and guidance as needed for medical patientsIncorporate "best practices" on the Medical UnitMaintains relevant policies and procedures, patient education, outcomes data collection, quality improvement efforts and education of relevant staff with focus on patient safety.Collaborate with Hospitalists, Primary Care Physicians, Surgeons and marketing department for educational sessions, community events and patient symposiums.Provides education to the patients and families regarding their hospital stay and discharge instructions.Acts as a liaison between patient, physicians' offices and nursing units for delivery of care.Provide education as needed to nursing staff.Responsible for helping with medical patients in staffing 1-2 days/week.Responsible for contacting patient and following them through their plan of care for compliance, questions, assistance with medical terminology, etc.Ability to interact directly with physicians, patients, referring physicians and hospital departments. Responsible for working in conjunction with ACNO to plan patient retention strategies.Must possess strong time management skills to meet time sensitive deadlines.Responsible for triaging physician concerns brought to employee either formally or informally to department Manager in a professional and timely manner for follow up.Responsible for complying with all applicable hospital and departmental policies and procedures, including, but not limited to: all departmental and functional policies and procedures.Responsible for supporting the goals and objectives of the department.Attends pertinent and assigned committee meetings.All other duties as assigned.Education RequirementsMinimum: Associates DegreePreferred: Bachelors degreeOther: RN CertificationExperience RequirementsMinimum: At least 2 years current Inpatient Medical hospital experience. One year of navigation or collaborative experience in health related field preferred.Preferred: Two or more years of navigation experience in a healthcare environment with experience working in a collaborative team model. RN CertificationLicense/Certification RequirementsState Issued RN License CPR - YesACLS - YesPALS - N/A
