Our Position

The Care Coordinator facilitates the quality and improvement of a person's experience of care, which may include admission, discharge, outreach, engagement, recovery, transition and case management.

Your Responsibilities

* Provides coordination of care of services and resources available internally as well as throughout the community. * Acts as an advocate for the needs and rights of individuals. * Coordinates or facilitates multidisciplinary team meetings to ensure services are provided in a timely manner. * Assists individuals in daily living activities and establishing and maintaining linkages that provide education, support and independence in the community. * Partners with other provider agencies in the coordination of care to help participants achieve wellness. * Documents relevant and required information and services assuring coordination of care as a tool for communication with other team members. * Understands and adheres to RI International compliance standards as they appear in the Code of Conduct and Conflict of Interest Policies. * Performs other related duties as assigned.
