Our Position

The Care Coordinator facilitates the quality and improvement of a person's experience of care, which may include admission, discharge, outreach, engagement, recovery, transition and case management.

Your Responsibilities

Provides coordination of care of services and resources available internally as well as throughout the community. Acts as an advocate for the needs and rights of individuals. Coordinates or facilitates multidisciplinary team meetings to ensure services are provided in a timely manner. Assists individuals in daily living activities and establishing and maintaining linkages that provide education, support and independence in the community. Partners with other provider agencies in the coordination of care to help participants achieve wellness. Documents relevant and required information and services assuring coordination of care as a tool for communication with other team members. Understands and adheres to RI International compliance standards as they appear in the Code of Conduct and Conflict of Interest Policies. Performs other related duties as assigned.

Ability to provide courteous assistance in a concise, informative manner, keeping a positive tone and appropriate non-verbal language. Bachelor's Degree in a behavioral health field and a minimum of one (1) year of behavioral health experience; or Bachelor's Degree in any field and a minimum of two (2) years of behavioral health experience; or High School Diploma or equivalent and a minimum of four (4) years of behavioral health experience. Ability to maintain a high level of confidentiality (HIPAA guidelines and regulations) and professionalism. We are a Second-Chance Employer Must be able to pass a seven-year background check. Must be able to pass substance abuse testing and/or tuberculous testing (if applicable). Must be able to pass education verification check. Must be able to pass three-year motor vehicle report and meet minimum eligibility requirements criteria of insurance carrier (if driving is required for this position).
