We are a small children's acute care hospital focused on treating complex medical conditions, chronic illnesses, developmental disabilities, and abuse and trauma. Our nurses are focused on providing exceptional patient care to children while helping families face unique challenges. As a Service Coordinator, you will help answer families' concerns about their infants and toddlers' development by facilitating developmental evaluations. The Service Coordinator works in partnership with developmental specialists. This includes speech therapists, physical therapists, occupational therapists, social workers, and developmental therapists. You will help families link to the support their children need to develop to their greatest potential. You will work with children until they reach three years old and then help the family transition to a preschool or special education program.Essential Service Coordinator-Social Worker Functions:Completing Intakes, Coordinating eligibility determination processAssisting families in locating service providersFacilitating development of individual family service plans (IFSP)Maintaining early intervention recordsMaintaining up to date accurate information on Cornerstone system,Maintaining on-going contact with families, Facilitating six-month IFSP reviewsImplementing transition planCompleting the family fee scheduleConducting developmental screenings and public awareness presentationsWhat we need from you:Bachelor's degree in Social Work, Psychology or related field requiredMinimum of 1 - 3 years working either in Early Intervention or with families of children with special needs requiredMust become credentialed as a Service CoordinatorEmployee must maintain automobile insuranceWhat you will receive: Competitive SalaryMedical, Dental, and VisionPet InsuranceLife & Disability InsuranceRetirement Savings Plans & MatchingGenerous Paid Time Off & HolidaysFree ParkingFree breakfast and lunch on the last Friday of each monthWho we are:La Rabida is a place unlike any other.
