Key Responsibilities:

  • Develop and maintain comprehensive social histories and psychosocial assessments for each Guest.
  • Orient Guests and families to our facility, its services, and their rights.
  • Attend care conferences and identify psychosocial needs for inclusion in the care plan.
  • Assist with coordination, intake, admission, and Guest transfer within the facility.
  • Discuss facility rules and regulations with Guests' families and respond to questions.
  • Coordinate the inspection of Guests' belongings and ensure they are properly labeled and inventoried.
  • Attend Interdisciplinary Team and Unit Care conferences, assess Guests for psychosocial groups, and supervise implementation and documentation.
  • Complete mandatory documentation within established timelines.
  • Provide consultation to Guests on a regular or as needed basis to discuss problems and assist with implementation of solutions.
  • Mediate issues that arise among Guests, families, and staff.
  • Provide linkage with community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs.
  • Develop comprehensive discharge plans in collaboration with Guests, families, multidisciplinary staff, and referral agencies.
  • Provide informal counseling to uncover problems that might interfere with Guests' socialization and participation in activities.
  • Establish, develop, maintain, and update filing systems for the Social Services Department.
  • Report hazardous conditions, damaged equipment, and supply issues to appropriate persons.
  • Follow established safety precautions when performing tasks and using equipment and supplies.
  • Maintain the comfort, privacy, and dignity of Guests and interact with them in a manner that displays warmth, respect, and promotes a caring environment.
  • Communicate and interact effectively and tactfully with Guests, visitors, families, peers, and supervisors.
  • Respond to call lights promptly and courteously when working in Guest care areas.
  • Attend and participate in departmental meetings and in-services as directed.

Requirements:

  • Two years' experience in a social worker role, preferably in a healthcare setting.
  • Bachelor's degree in social work or a similar field.
  • 5 years related experience.

Work Environment:

This role operates in a healthcare setting and requires regular walking to various locations around the facility. The role may also come into contact with Guests who may have contagious illnesses.

Physical Demands:

This role requires the ability to lift files, open filing cabinets, and bend or stand as necessary.

Travel:

This position may travel to and from various locations in and around the surrounding community.
