The Health Services Coordinator position will support processes within the Health Services department, including managing pharmacy requests and communications; analyzing health services activities and data; following up on the submission of fully signed RN Summaries; tracking health care action steps; monitoring increase unit requests and authorizations; and supporting management trainings as needed. Essential Functions: * Gathers, organizes and analyzes data for Health Services activities. Utilizes EHR and other systems to monitor and report data as needed. * Manages pharmacy-related requests and communications, and follows up with pharmacy and CL team to prevent medication errors and ensure accurate medication administration. * Supports CL team with RN Summary completion. Tracks and follows up on all required signatures and ensures RN Summaries are submitted for retention. * Supports the Community RN team with the Health Risk Screening Tool (HRST) process. Tracks when HRSTs are coming due and when they are completed, and gathers information related to the process. * Tracks additional unit requests and authorizations. Monitors the process to ensure that that additional unit requests are handled in a timely manner. * Updates and maintains health services documents, including ensuring that up-to-date versions are available on shared drives. * Supports management trainings and 1:1 refresher trainings regarding health services procedures as needed. Position Competencies: * Knowledge of disabilities, medical care and behavioral supports. Participates in organizational and funder trainings, and external training opportunities for ongoing growth and development. * Ability to multitask and prioritize activities daily to ensure timelines are met. * Establishes and maintains successful interpersonal relationships and works cooperatively in group situations. * Intermediate-level proficiency with Microsoft Office products and computer systems. Qualifications: * Bachelor's degree in human services or related field strongly preferred. May consider equivalent combination of education and experience. * Must have at least 1 year of experience working with individuals with disabilities. * Must have at least 1 year of experience managing a project, system or caseload. * Must have experience using the Microsoft Office suite and intermediate-level skill with Excel. * Previous experience working with Employee Health Record systems or other data systems preferred.
