You will serve as a key liaison between clients, their families, and our care team, ensuring that our clients receive the best possible care and support.

Qualifications:

  • Excellent problem-solving abilities and a proactive approach to addressing client needs.
  • Ability to work independently as well as part of a collaborative team.
  • Proficiency in using care management software and Microsoft Office Suite.
  • Valid driver's license and reliable transportation.
