Job DescriptionPatient Care Coordinator - Kadlec Regional Medical Center - Richland, WASupports the implementation, management, and support of KRMC & KC Medication Assistance Program; Responsible to carry out the daily workflow, such as: Conduct patient interviews, Support patient financial applications and review for completeness, Assign level of poverty/access to medication assistance, Document in EPIC, Receive and encourage referrals for patient needs/access to care, Work within and facilitate manufacturer assistance programs, Develop workflows to efficiently and effectively track, monitor, support, evaluate, and direct the appropriate application of the defined policies and procedures supporting the Medication Assistance Program.Providence caregivers are not simply valued - they're invaluable.
