Duties and Responsibilities:
Provide oversight/guidance of Antimicrobial Stewardship Program

Develop and provide pharmacy in services on a set schedule e.g. on new drugs to physicians, residents, nursing, clinical staff to advocate and increase awareness of Pharmacy Dept/practice/advancements
Coordinate and lead P&T in partnership w/ Director & Infection Control
Continually keep updated with clinical guidelines, standard of practice to create new and update existing clinical policies and educate pharmacy staff and clinicians
Develop and inform clinical staff drug shortage clinical strategies in partnership with Pharmacy Ops
Investigational Drug Services Program Development, Training, Maintenance, Expansion
Employee must perform all duties and responsibilities in accordance with the C-I-CARE Standards of the Hospital. C-I-CARE is the foundation of Stanford Healthcare - ValleyCares patient-experience and represents a framework for patient-centered interactions

Education Qualifications

Graduate of an ACPE-accredited School of Pharmacy
Doctor of Pharmacy degree and Clinical Pharmacy residency preferred or equivalent practice experience
Completion of PGY1 residency

Licenses and Certifications

Current American Heart Association BLS Certification
Licensed/Registered Pharmacist in the state of practice, California.

Experience

8-10 years health-system experience preferred with 3-5 years in a clinical pharmacist position required
At least 3-5 years of supervisory and/or advance education/training preferred{advertiserId% %{category%%
