Job Summary:

As a clinical case manager, you will be responsible for overseeing the treatment and care of residents, ensuring they receive comprehensive support and services. Your role involves assessing resident’s needs, developing care plans, coordinating with Louisville Metro Housing Authority (LMHA) staff, and advocating for residents to ensure they receive appropriate resources and treatments.

Key Responsibilities:

1. Assessment and Care Planning:

  • Conduct comprehensive assessments of residents medical, psychological, and social needs.
  • Develop individualized care plans in collaboration with residents, families, and healthcare providers.
  • Monitor and evaluate the effectiveness of care plans and adjust, as necessary.

2. Coordination of Care:

  • Coordinate services and resources to ensure seamless delivery of care.
  • Facilitate communication between LMHA, and residents to promote continuity of care.
  • Arrange for referrals, community resources, and support services as needed.

3. Advocacy and Support:

  • Advocate for residents rights and access to services.
  • Provide emotional support and education to residents about treatments, and resources.
  • Assist residents in navigating support programs.

4. Documentation and Reporting:

  • Maintain accurate and up-to-date documentation of resident assessments, care plans, and progress notes.
  • Ensure compliance with regulatory requirements and organizational policies.
  • Prepare reports on resident outcomes, utilization of services, and other relevant metrics.

5. Collaboration and Teamwork:

  • Collaborate with interdisciplinary teams.
  • Participate in case conferences, meetings, and quality improvement initiatives to enhance resident care and outcomes.

Required Qualifications:

  • Bachelor’s or Master’s degree in Social Work or a related field.
  • Licensure or certification as required by state regulations
  • Proven experience in clinical case management.
  • Strong assessment, communication, and organizational skills.
  • Knowledge of resident care protocols.

Preferred Skills:

Experience working with specific populations (e.g., children, elderly, individuals with disabilities, mental health, and substance misuse disorders).
