Key Responsibilities

As an RN Case Manager, you will be instrumental in the Care Coordination Department, executing the following duties:

  • Conduct thorough initial assessments to optimize clinical resource management, ensuring appropriate hospital care levels.
  • Engage in ongoing multidisciplinary evaluations to maintain high standards of patient care.
  • Develop and adjust individualized care plans based on patient needs and interdisciplinary collaboration, ensuring accurate documentation in the electronic medical record (EMR).
  • Facilitate patient discharge planning in coordination with relevant internal and external providers while safeguarding patient confidentiality.
  • Maintain precise and professional documentation as part of the Quality Review Audit process.
  • Advocate for patients and families with respect and confidentiality, addressing their needs effectively.
  • Act as a liaison between patients and the healthcare team, educating staff on the RN Case Manager's role and providing resources as needed.
  • Refer complex cases to appropriate management when necessary, ensuring all patient care needs are met.
  • Deliver essential discharge documentation to patients as required.
  • Address and complete consultations related to case management in the EMR.

Additional Responsibilities:

  • Collaborate with social workers to identify and address social and financial challenges faced by patients.
  • Make referrals to social services for patients exhibiting high-risk indicators.
  • Participate in departmental meetings and committees to enhance team communication and project involvement.
  • Engage in continuous professional development through educational opportunities and literature review.
  • Provide guidance to physicians and staff regarding payer issues, documentation, and compliance matters.
  • Support physicians in clinical management and operational efficiency.
  • Adhere to all organizational policies and standards of conduct.
  • Ensure compliance with HIPAA regulations to protect patient privacy.
  • Perform other related duties as assigned.
Qualifications

Required Qualifications:

  • Current licensure or eligibility for licensure in Kentucky.
  • Basic Life Support (BLS) certification from the American Heart Association (AHA).
  • Bachelor's degree in nursing or related field preferred.
  • A minimum of 5 years of experience in care management within an acute care setting, with strong organizational and communication skills.

Preferred Qualifications:

  • Certification in a nursing specialty or Certified Case Manager (CCM) designation.
