As a Care Coordinator, you will be an essential member of our multidisciplinary team, providing comprehensive care coordination, treatment, and support services to our clients. If you are looking to make an impact in the community and jumpstart your career with a supportive team environment, apply today Primary Responsibilities: Provide direct care coordination services to an assigned group of clients, including individual, group, and family counseling, crisis intervention, and case management.Collaborate with the treatment team to develop, implement, evaluate, and revise treatment goals and plans for clients.Educate and support clients' families, advocating for clients' rights and preferences. Coordinate with community agencies and systems to ensure holistic treatment.Assist clients in finding primary care physicians, dentists, and coordinating psychiatric care with medical care.Offer crisis intervention counseling and collaborate with Designated Crisis Responders when necessary.Maintain thorough and accurate documentation of client progress according to established policies.Participate actively in organizational meetings and treatment planning review meetings.Provide information and support to clients regarding substance use issues, mental and physical health, and daily functioning.Help clients access resources and education for increased autonomy and meeting basic needs.Monitor and facilitate Legal Restraint Alternatives (LRA) compliance for assigned clients, ensuring continuity of care.Collaborate with the Crisis Response Team (CRT) and court-involved personnel for LRA extensions and revocations as needed.Minimum Qualifications: EDUCATION: Bachelor's Degree in Psychology, social services, or behavioral health field. EXPERIENCE: Entry level (no prior related work experience) This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must successfully complete a motor vehicle history check, possess and maintain a current, valid driver's license in the state of Washington, and have reliable, insured transportation. LICENSURE: Agency Affiliated Counselor Registration. Preferred Qualifications: EXPERIENCE: Experienced (minimum 2 years of job-related experience) Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment.
