The Mental Health Association of Columbia-Greene Counties Inc. is looking to fill the Enrollment Care Coordinator position. Monday-Friday 8:30am to 4:30pm at an hourly rate of $20.879.

***This position offers some flexibility to work remotely at the discretion of the program manager and program needs.***

Care Coordination provides services to both Columbia and Greene counties. Care Coordinators provide coordination of behavioral and/or physical health care for any person who is Medicaid eligible, regardless of diagnosis. Care Coordinators can help those who qualify to attain the best possible physical and/or mental health services. Care Coordinators are contracted through our Health Home (Hudson River Health Care) to provide these services. Individualized wellness plans in coordination with other providers' plans are developed to help individuals manage a healthier day-to-day life, with a focus on rehabilitation, recovery, and clinical service linkage.

The work is sedentary. Typically, the employee may sit comfortably to do the work, with some walking; standing; bending; carrying of items such as papers, books, small parts; driving an automobile; etc. No special physical demands are required to perform the work: Will be required to sit/stand for extended periods of time.

Will use a computer to enter data.

Will be required to sit, talk, or hear frequently.

Will be required to drive to provide the outreach in the community.

Will be required to use repetitive hand motion, handle or feel, and to stand, walk, reach, bend or lift up to 20 pounds occasionally.

Will be required to travel to other locations and come in contact with the public which may risk exposure to irrational/hostile behavior.

The physical environment may require the employee to work both inside and outside in heat/cold conditions.

Requirements

Work is primarily performed independently, and initiative is expected on specific aspects of the work except under unusual circumstances or conditions. Procedural guidance is available.

Monitor outreach assignments on a monthly basis from the Health Home.

Monitor all incoming community referrals and submit to Health Home for approval.

Outreach members about Care Coordination to schedule enrollments into the program.

Conduct enrollment appointments and obtain necessary consents and paperwork for program admission.

Carry a limited caseload.

Monitor and update electronic health records as necessary.

Be fluent in Medicaid eligibility and restriction eligibility codes.

Engage with the local hospital on members coming out of the hospital.

Any and all other duties as deemed necessary by the Program Director of Care Coordination for the success of the program. Must have a high school diploma, and must have 2+ years experience providing direct services to people with Serious Mental Illness, developmental disabilities, alcoholism or substance abuse, and/or children with SED, and/or linking these populations to services essential to successful living in the community setting.

Solid knowledge of an office environment, quality, customer service, direct care and service procedures.

Requires effective communication skills. Communication involves frequent contacts with clients, staff, program management, vendors and regulatory agencies in a managerial capacity. Represents the organization to outside agencies, vendors, and stakeholders, Communication is typically non-controversial in nature and may be internal and external. Must be able to handle communications in a sensitive manner and with diplomacy.

Ability to read, analyze, and interpret business publications, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

Proficiency with online technology systems, including electronic health records.

Willingness to perform work in the field, visiting clients as needed.

Knowledge of managed care as well as familiarity with Greene County is preferred.

Must have a valid NYS Driver's License and a clean MVR, and reliable transportation.

Must be self-motivated.

Must have excellent verbal, written, communication, and interpersonal skills.

Must be able to obtain a Statewide Central Registry and Fingerprint clearance.

Regular and steady attendance is a requirement for all positions.
