The Care Coordinator assists patients at Glenbeigh to help provide a stream lined discharge plan covering multiple facets of recovery and assists in improving recovery outcomes. The Care Coordinator participates in a multidisciplinary team to assist in providing care to patients and to review the potential discharge challenges of patients. The Care Coordinator assists counselors, counseling assistants, and patients as needed and is a critical component of the overall treatment experience at Glenbeigh. Education, Knowledge, Skills and AbilitiesKnowledge of addiction, treatment and the recovery process.Maintain a good physical/emotional well-being, a positive attitude and role model.Strong organizational skills and effective in working with a team.Excellent verbal/written and interpersonal communication required.Able to maintain a strict level of confidentiality of client issues and agency matters and abide by all HIPAA guidelines.Ability to perform multiple tasks simultaneously and maintain client chartingBasic computer skills including but not limited to: Microsoft office, Outlook, Word, Excel, Teams. Experience utilizing an electronic medical record preferred.Basic office equipment skills including but not limited to: photocopiers, scanners, fax machines, telephone system, shred boxes, calculators, computers, printers and keyboards. Recovering persons must have a minimum of one (1) year of sobriety.High School diploma or equivalentRequired Length and Type of ExperienceOne year experience in the substance abuse field preferred.Required Licensure or CertificationAssociate's degree in social work, psychology, ore related field or in the process of obtaining. Must have a minimum of CDCA licensure of in process of obtaining.Required Physical and Environmental DemandsThis job also requires that an individual be able to work under periods of high stress and in a consistently fast-paced environment.Stretching, bending, lifting, extensive walking throughout hospital grounds and/or buildings, repetitive hand motion, sitting, standing and spending considerable amount of time sitting at a desk or computer terminal.
