ADvantage Case Manager

With required certification and training, provide certified ADvantage Case Management services though collaborating with ADvantage Members and their support systems, including family members, caregivers, etc., by assessing, facilitating, planning, advocating and monitoring health and social needs, thus enabling members to remain in their home and in the community. Administer certified ADvantage case management services within the scope of practice of the LIFE Senior Services certified program. Work may entail working with high risk clients and helping mentor new certified ADvantage case managers, as appropriate.

Requirements

EDUCATION AND EXPERIENCE: BA, BSN, MA or MS in social sciences, health or any related field with professional experience in case management; current Registered Nurse registration with professional case management work experience. All ADvantage Case Managers must be certified by the State and have one year prior professional experience working with seniors and/or people with disabilities.
