Job Title: Registered Nurse - Patient Care Coordinator
Job Specialty: Critical Need Care Management
Job Duration: 13 weeks
Shift: 8-hour shift from 06:00 AM to 02:00 PM
Guaranteed Hours: 40 hours per week
Experience: Minimum years of experience required
License: Active RN license
Certifications: Relevant certifications required

Must-Have:
- Active RN license
- Experience as a Patient Care Coordinator
- Strong communication skills
- Ability to work in a fast-paced environment
- Knowledge of care management protocols

Job Description:
- Serve as the Patient Care Coordinator in a critical need care management unit
- Coordinate patient care activities and ensure smooth patient flow
- Work closely with the care management team to provide high-quality patient care
- Collaborate with healthcare professionals to develop and implement patient care plans
- Monitor patient progress and update care plans as needed
- Ensure compliance with care management protocols and regulations
- Provide support and guidance to patients and their families
- Maintain accurate and up-to-date patient records
- Participate in care management meetings and contribute to care improvement initiatives
