Summary:

The Nurse Navigator serves as a consistent care coordinator for the diagnostic and surgical end of Urologic care to ensure timely access and seamless coordination of services throughout the continuum. The Nurse navigator will interface with the schedulers, internal and external providers, and assist in the diagnostic imaging, clinical evaluation, support services, and education of patients receiving services. The nurse navigator will be responsible for facilitating and coordinating the care of patients of USME that receive their care from providers working in collaboration with USME. He or She will identify and help eliminate barriers and refer to appropriate resources. Additionally, the nurse navigator will be responsible for the triage and coordination of referrals sent to our office. They will coordinate local support systems, arrange patient focus groups, and collaborate and provide educational resources for patients, families, and staff. They will be responsible for assisting with clinical needs and acting as a resource for clinical staff.

  • Uses a constructive consistent approach in the oversight of staff, in providing feedback on the delivery of patient care and in the maintenance of positive interdepartmental relationships
  • Serves as a support to staff delivering services to patients
  • Ability to generate effective staff responses to needs identified via patient satisfaction scores and feedback
  • Able to manage physician needs to expedite patient care
  • Provides follow-up on patient, family and physician complaints when requested by the Department Head
  • Develops and maintains positive working relationships and working environment
  • Efficiently coordinates scheduling of staff
  • Provides input on staff performance to the Nurse Manager and/or timely performance appraisals
  • Assists Department Head with counseling and disciplining of staff, monitoring corrective action/performance improvement when required
  • Adheres to job description and established performance standards
  • Serves as a resource to all members of the health care team
  • Able to perform all the tasks of Staff Nurse
  • Develop and implement triage standards to include proper telephone etiquette
  • Collaborates with other disciplines based on patient assessment and identified needs
  • Completes assigned or requested tasks in a timely, efficient manner
  • Coordinates patient activities within the department and/or among departments
  • Actively works to creatively problem solve to meet service and program needs (patient, family, staff, and physician)
  • Provides day to day supervision of clinical operation and personnel performance
  • Assists in coordination of educational opportunities for staff development
  • Provides outreach and community education as requested
  • Develops and maintains through effective collaboration a competency based orientation program
  • Coordinates the development, implementation and evaluation of patient/family education programs
  • Performs all other duties as assigned
  • Demonstrates clinical expertise and high quality standards in all aspects of clinical care.
  • Enhances departmental delivery of quality patient care
  • Monitors patient care assignments for efficiency and effectiveness
  • Upholds all mandated regulations and monitors staff compliance
  • Ensures personnel are competent in their performance, compliant with policy and procedures and practice within the scope of their job description and/or license
  • Oversees employee adherence to infection control, safety, emergency, policy and procedure, and incident reporting standards
  • Ensures appropriate documentation is maintained in the medical record
  • Appropriately identifies concerns and reports unit activities to Department Head
  • Makes suggestions for and encourages staff to participate in process / quality improvement
  • Generates and updates unit policies and procedures according to current national standards for clinical practice
  • Coordinates the development of disease specific, treatment, and navigation pathways design
  • Works to implement feasible process / quality improvement suggestions
  • Actively participates in both unit and hospital wide performance improvement initiatives including audits and quality reports
  • Assists in development and implementation of quality assurance/ performance improvement monitors
  • Coordinates, supervises, and evaluates of care given to all patients.
  • Attends 100% of mandatory in-services
  • Initials 100% of mandatory memos and staff meeting minutes
  • Attends 75% of all staff meetings
  • Works to enhance delivery of quality cost effective care
  • Participates in unit goal setting and implementation
  • Provides ongoing support for established goals and objectives
  • Participates in problem solving team to address patient flow issues
  • Able to address workflow or patient issues effectively with physician input
  • Maintains specialty certification
  • Maintains unit specialty certifications/competencies
  • Earns a minimum of 20 CEUs per year in addition to mandatory clinical requirements. Note: 1 credit hour = 15 CEUs
  • Maintains competency and supports process changes with the electronic medical record.
