Nurse Navigator/RN

The Nurse Navigator is a key member of the Urology Clinic team, responsible for coordinating patient care and ensuring seamless transitions throughout the diagnostic and surgical process. This role requires strong communication and organizational skills, with a focus on delivering high-quality patient care and promoting a positive patient experience.
Key Responsibilities:
- Coordinate patient care and ensure timely access to services
- Interface with schedulers, internal and external providers, and assist with diagnostic imaging, clinical evaluation, and support services
- Identify and eliminate barriers to care, and refer patients to appropriate resources
- Coordinate local support systems, arrange patient focus groups, and provide educational resources for patients, families, and staff
- Assist with clinical needs and act as a resource for clinical staff
Requirements:
- Registered Nurse (RN) license
- Associate's degree in Nursing
- Strong communication and organizational skills
- Ability to work effectively in a fast-paced environment
- Knowledge of urology and patient care principles
