As a Manager, Care Delivery and Innovation, you will be responsible for working across our team at Hopscotch to understand the priorities and needs of the business as we build clinical programs and drive innovation to deliver clinical, operational, and financial results.

Our ideal candidate will bring skill, experience, tenacity and passion to play an instrumental role to codify the strategy, manage a detailed work plan to ensure execution and accountability and build strong, trusted relationships across the central and clinical teams to enable results and success.

This role is based in Chicago, IL and will require some travel to the market, initially North Carolina, on an ‘as needed’ basis to enable collaboration, relationship building and execution against plans.

WHAT YOU’LL DO Specific responsibilities for this role will include, but are not limited to:

Support the design, build, and execution of our Specialty Care strategy

Own the development of key clinical programs to deliver best-in-class patient outcomes

Collaborate with Senior Director, Care Delivery and Innovation to continuously review and refine Transitions of Care Program

Work cross-functionally to monitor clinical programs performance and make recommendations for process improvement

Develop and manage programs to address social determinants of health (transportation)

Partner with Population Health and Operations team to implement and monitor performance of programs

Collaborate with Product team to develop tools to optimize workflows related to disease management

Analyze data and recommend areas of opportunity for clinical performance

Other duties and projects as assigned

ABOUT YOU We are looking for a motivated, mission driven, and experienced leader with:

3-5 years of relevant experience in healthcare, population health, clinical programs, working with providers, or other similar experience with strong preference for value-based care models

Bachelor’s Degree required

Clinically strategic to develop robust insights from data and analysis, with an ability to translate findings into action

Ability to lead and execute on projects

Experience working with multiple software & data platforms, electronic health records, spreadsheet development and navigation, data processing, organizational skills and ability to manage multiple priorities appropriately

US work authorization
